Approved Documentation

Applicants who aren't able to supply a valid ID must present 2 forms of documentation, which could include:
  • Bank statement
  • Current vehicle registration
  • Lease / rental agreement
  • Notarized letter
  • Pay stub
  • Social Security card
  • Tax return / W-2 Form
  • Utility bill
At least one of those documents must contain the applicant's current mailing address and name.

Applicants that cannot provide a government-issued photo identification will not receive the birth certificate the same day. The certificate will be mailed to the address displayed on the documentation submitted.
