DIABETES & HIGH BLOOD PRESSURE
EDUCATION AND COACHING
The Frederick County Health Department's I'm Living Healthy Program provides educational group classes and individual coaching sessions "at no cost" to Frederick County residents that have or are at risk for diabetes and/or high blood pressure.
What Will I Receive?
Weekly education and coaching with a registered nurse and health educator for up to 8 sessions
The knowledge and skills you need to accomplish your health goals
Connections to community resources
What Makes I'm Living Healthy Different?
I'm Living Healthy is not a standard classroom-style program. It requires you to:
Be open to new ideas & lifestyle changes
Work as a teamwith your nurse and health educator
