Adult Evaluation & Review Services (AERS)

AERS, formerly known as Geriatric Evaluation Services, is an in-home evaluation and assessment program for aged and/or disabled adults. The STEPS evaluation is completed in the home by a nurse or social worker. The evaluations help to make recommendations and identify resources that help the aged and/or disabled adults who are at risk for nursing facility placement to remain living in the community.

AERS also performs inter-RAI evaluations for community Medicaid recipients who are seeking Home and Community-based Services (HCBS).

AERS is responsible for performing the federally mandated PASRR evaluations for clients who have an Intellectual Disability (ID) or Mental Illness (MI).

  • To help aged or disabled persons in our community continue living independently, if possible, with the utilization of community resources.
  • To reduce unnecessary admissions and length of stay in psychiatric and long term care facilities.

Eligibility

STEPS: Aged and/or Disabled adults at risk for nursing facility placement who wish to remain living their homes/community that do not have full Community Medical Assistance Benefits.

Inter-RAI: Adults or children with active full Medical Assistance Benefits.

PASRR
: Any adult who has an intellectual disability or mental illness entering into a Nursing Facility.
