Death Certificates
You can get copies of death certificates for close family members if the death happened on or after January 1, 2015, and the death certificate is registered.
If the death was before January 1, 2015, you need to call the Division of Vital Records at 410-764-3053 to get the death certificate.
There are two ways to apply:
- In person Monday- Friday 8:30am - 4:00pm. Please note that we are closed for lunch from 12:00 pm - 1:00 pm.
- You can fill out the Death Certificate Application (English/Español) and bring it with you or get a copy when you arrive.

A person who is related to the deceased must show proof of their relationship with forms like:
- Birth Certificate
- Marriage Certificate
- Obituary
An authorized representative of a relative of the deceased will need forms like:
- Letter of Administration
- Signed by the Court Clerk or the Register of Wills
OR
- Authorization of Release
- Signed by the living relative
- Must include a copy of the living relative’s valid ID
- Must include name of deceased
- Must include relationship of living relative to deceased
If you are making an insurance claim or need it for business, you must provide documents such as:
- Letter from the insurance company
- Deed
- Vehicle Title
If required by a court, provide a copy of the court order.
The cost is $18 for the first death certificate and $20 for each additional death certificate. We accept cash, check, Discover, MasterCard or Visa credit cards.
