Assessments, Evaluation & Review Services (AERS)
AERS, is an in-home evaluation and assessment program for aged and/or disabled adults. The evaluation is completed in the home by a nurse or social worker. The evaluations help to make recommendations and identify resources that help the aged and/or disabled adults who are at risk for nursing facility placement to remain living in the community.
AERS also performs evaluations for community Medicaid recipients who are seeking Home and Community-based Services (HCBS).
AERS is responsible for performing the federally mandated pre-admission screening and resident review (PASRR) evaluations for clients who have an Intellectual Disability (ID) or Mental Illness (MI) for people entering a long-term care facility.

Eligibility
STEPS: Aged and/or Disabled adults at risk for nursing facility placement who wish to remain living their homes/community that do not have full Community Medical Assistance Benefits.
Inter-RAI: Adults or children with active full Medical Assistance Benefits.
PASRR: Any adult who has an intellectual disability or mental illness entering into a Nursing Facility.
AERS offers services at no cost.

The interRAI assessment is completed for community Medicaid recipients who are seeking Home and Community-Based Services (HCBS) such as the Community First Choice (CFC), Community Personal Assistance (CPAS) and Community Options Waiver Programs. The Nurse Monitor is responsible for monitoring the client’s Residential Service Agency (RSA) once the client is receiving CFC/CPAS/COW services.
Eligibility: Adults or children with active full Medical Assistance Benefits.
Preadmission Screening and Resident Review (PASRR) is a federal requirement to help ensure that individuals with an intellectual disability (ID) or a serious mental illness (SMI) are not inappropriately placed in nursing homes for long term care. PASRR requires that Medicaid-certified nursing facilities:
- Evaluate all applicants for serious mental illness (SMI) and/or intellectual disability (ID)
- Offer all applicants the most appropriate setting for their needs (in the community, a nursing facility, or acute care settings)
- Provide all applicants the services they need in those settings.
The PASRR evaluation is used for the purpose of preventing unnecessary/inappropriate nursing facility admission or retention of those with an Intellectually Disability (ID) or a Severe Mental Illness (SMI) diagnosis.
Eligibility: Any person who is entering a Skilled Nursing Facility (SNF) that has an ID or SMI diagnosis must be screened.
AERS provides short term case management to AERS patients. The AERS RN Case Manager receives referrals from the AERS assessors and is responsible for contacting the clients and matching community resources to the recommendations in the patient's Plan of Care (POC). Only current AERS patients are eligible for short-term case management.
Statewide Evaluation and Planning Services (STEPS): Rework
The STEPS evaluation is a free evaluation that is completed in the home by a nurse or social worker. The evaluations help to make recommendations and identify resources that help the aged and/or disabled adults who are at risk for nursing facility placement to remain living in their own home.
Eligibility: Aged or disabled adults at risk for nursing facility placement who wish to remain in their homes/community that do not have full active community Medical Assistance Benefits.
