Special delivery nursing and home visiting program is a free service that exists to promote the health of pregnant women and their infants through education and resource referrals on a person to person basis.
We inform our clients in order to empower them to make the best choices for health. Admission to this program is limited to Medical Assistance eligible Frederick County pregnant women who have 1 or more risk factors as identified by their Obstetric (OB) provider.
Women enrolled in this program are phone case managed, will meet their RN case manager at least once during the pregnancy and can expect a postpartum home visit after giving birth.
