Notice: Frederick County Health Department Mental Health Services Clinic is Closed

Effective August 1, 2025, the Frederick County Health Department Mental Health Services (MHS) has closed their clinic at 350 Montevue Lane, Frederick, MD 21702 and 300 A South Seton Avenue, Emmitsburg, MD 21727.  Your medical record can be sent to the provider of your choice, at no charge. You may also request a personal copy of your record, for a fee. To allow us to release your record to a provider or to you, you will need to complete an “Authorization to Release” form.  A form can be mailed to you upon your request or you can come into MHS to complete the form.  Return the form or complete it in person before July 30, 2025, for the fastest service. Your record will be stored at the Frederick County Health Department until it is due to be destroyed based on State law.  Please contact MHS on 301-600-1755 with any questions. To find a new mental health provider you may call MHS (301-600-1755) for a list of community providers, contact your insurance, or call 211 for a referral to providers in the area.
